This template supports allied health professionals to document, reason, and communicate risk clearly when prescribing assistive technology or home modifications under the Aged Care Act 2024 and Support at Home program.
It provides a structured way to record discussions, identify and mitigate risks, and uphold dignity of risk while ensuring the safety of clients, carers, and support workers.
